Work Permit

Work permits are obtained through the main office.  Stop in and get the form or download it from this site.  Fill out the form making sure to have the parents signature on the bottom, once it is filled out bring it to the office with a document that proves the date of birth. ex: (Birth Certificate, drivers license, permit, passport etc).  After verifying the DOB we will issue the work permit that you will need to show your employer.
